Meet Samantha Carry, APRN, FNP-C


Samantha is a Certified Family Nurse Practitioner, with advanced training in Functional Medicine. She is passionate about helping people get to the root cause of their health concerns. She is especially passionate about women’s health (from reproduction to menopause), metabolism, and healing chronic disease.

Comprehensive Health Timeline: We map out your entire health history—from birth to the present—identifying the "triggers" (the suspected events that may have started your symptoms) and "mediators" (the factors that may be preventing your optimal health).

  • Lifestyle & Environmental Assessment: We analyze the "foundational five": your nutrition, sleep quality, exercise habits, stress levels, and environmental exposures (like toxins or allergens). In addition to the exploration of current symptoms, we prioritize the optimization of health and the prevention of disease.

  • The Functional Matrix: We use a systems-biology approach to see how different parts of your body—such as your gut, hormones, and immune system—are interacting and where any core imbalances may lie.

  • Advanced Diagnostic Strategy: In addition to standard labs, we may recommend specialized testing (such as gut microbiome, advanced hormone panels, or nutrient levels) to go deeper than standard "normal" blood work may go.

  • Collaborative Wellness Roadmap: You walk away with a personalized plan that includes any recommended labs, targeted nutrition, and lifestyle shifts tailored specifically to your body's needs.

Initial Intake Visit

Our initial intake is an in-depth session where we move beyond symptom management to perform a Root Cause Analysis. Instead of asking only "what" symptoms you have, we focus on "why" your body is out of balance. This visit is a collaborative investigation into your unique biology, history, and environment to find lasting solutions rather than temporary fixes.

Are Functional Medicine services covered by insurance?

Functional Medicine providers at Iris Mental Health and Wellness bill insurance directly for many of their services. The laboratories themselves may or may not directly bill your insurance. You may attempt to receive reimbursement after the completion of the labs from your insurance company (if it is a commercial plan that is not state medical assistance, Medicare or Medicaid) by requesting a Superbill from their provider and submitting that to your insurance company. You understand that you are financially responsible for such costs even if you have health insurance, depending on the benefits and coverage limitations of your health insurance policy.

Functional Medicine providers at Iris Mental Health and Wellness contract with insurance companies but do not guarantee insurance coverage for all services and products ordered and rendered. Functional Medicine products are non-refundable.

Functional Medicine providers at Iris Mental Health and Wellness do accept credit, debit and HSA/FSA accounts as payment. You understand that depending on your insurance coverage, you may be unable to obtain reimbursement for some services rendered, laboratory tests ordered, and medications or supplements received. Patients using their insurance will be responsible for any deductible, copay or coinsurance (percentage) of the negotiated rate with each insurance company, or for any remaining balance should your insurance not cover your visit. It is YOUR responsibility to verify insurance coverage and benefits.

Will I have to fill out paperwork?

Yes. After securing the appointment, you will receive an Intake Packet via the patient portal that needs to be completed no less than 48 hours before the appointment time to ensure that the Provider has the ability to review the information. Failure to do so will result in an automatic rescheduling of the appointment.
