Our initial intake is an in-depth session where we move beyond symptom management to perform a Root Cause Analysis. Instead of asking only "what" symptoms you have, we focus on "why" your body is out of balance. This visit is a collaborative investigation into your unique biology, history, and environment to find lasting solutions rather than temporary fixes.

Samantha is a Certified Family Nurse Practitioner, with advanced training in Functional Medicine. She is passionate about helping people get to the root cause of their health concerns. She is especially passionate about women’s health (from reproduction to menopause), metabolism, and healing chronic disease.

Comprehensive Health Timeline: We map out your entire health history—from birth to the present—identifying the "triggers" (the suspected events that may have started your symptoms) and "mediators" (the factors that may be preventing your optimal health).
Lifestyle & Environmental Assessment: We analyze the "foundational five": your nutrition, sleep quality, exercise habits, stress levels, and environmental exposures (like toxins or allergens). In addition to the exploration of current symptoms, we prioritize the optimization of health and the prevention of disease.
The Functional Matrix: We use a systems-biology approach to see how different parts of your body—such as your gut, hormones, and immune system—are interacting and where any core imbalances may lie.
Advanced Diagnostic Strategy: In addition to standard labs, we may recommend specialized testing (such as gut microbiome, advanced hormone panels, or nutrient levels) to go deeper than standard "normal" blood work may go.

New Patient Intake (Adult) - 90 minutes $475.00
The initial intake visit for functional medicine is significantly more comprehensive than a standard physical, focusing on the "why" behind your symptoms rather than just the "what." This process uses root cause analysis to look at the body as the interconnected system that it is. In this 90 minute session, we will examine your unique medical history, genetics, lifestyle and environment to create a roadmap back to health and vitality. By the end of the session, you will leave with a clear and guided plan forward, including recommendations such as individualized nutrition plans, proposed lifestyle accommodations and targeted lab orders.
New Patient Intake (Pediatric) 60 minutes $325.00

Established Patient Visit - 15 minutes $100.00
(Phone visit) This visit serves as a follow-up for patients with an established treatment plan. This visit is optimal for brief questions regarding a current treatment plan.
Established Patient Visit - 30 minutes $175.00
This visit serves as a follow-up for patients with an established treatment plan. This visit can also serve as a limited check-in with a small lab or imaging review.
Established Patient Visit - 45 minutes $250.00
This visit serves as a follow-up for patients with an established treatment plan. This visit is a helpful for patients with more than one treatment plan and/or anticipated changes to an established treatment plan. This can also serve as a both a routine check-in with a small lab/imaging review or a limited check-in with moderate lab/imaging review.
Established Patient Visit - 60 minutes $325.00
This visit serves as a follow-up for patients with an established treatment plan. This visit is a helpful for patients with more complex treatment plans, new concerns, and/or several questions regarding an established treatment plan. This can also serve as a both a routine check-in with a moderate lab/imaging review.

Functional Medicine providers at Bodhi Clinic/Iris Mental Health and Wellness value you as a patient and look forward to caring for you. When you book an appointment, the providers block their schedule for you and ask that you exercise the same consideration by adding the scheduled appointment to your calendar. We do not want missed appointment fees to be an impediment to your care; however, we need to ensure that providers are compensated for their time.
We require a cancellation notice of at least 48 hours (e.g. If your appointment time was 8:00 am on Monday, you have until 8:00 am on Wednesday to cancel or reschedule the appointment). If the appointment is cancelled or rescheduled within 48 hours, 100% credit will be applied towards a reschedule. If the appointment is cancelled with intent to not to complete the visit, the total cost of the visit less the credit card processing fees necessary to process that payment will be refunded to the original payment method.
There will be a cancellation fee of 50% of the cost of the visit if the appointment is cancelled or rescheduled before 48 hours of the appointment time. A 50% credit will be applied towards a rescheduled appointment.. If the appointment is cancelled with intent to not complete the appointment, 50% of the cost of the visit less the credit card processing fees necessary to process that payment will be refunded to the original payment method.
No-Show or a same-day cancellation fee is 100% of the cost of the visit.
If you are more than 15 minutes late past the scheduled appointment time, Bodhi Clinic/Iris Mental Health and Wellness considers this a same-day cancellation and no-show; therefore, the fee will be 100% of the cost of the visit. The Clinic and their Provider is not obligated to start the appointment at the time of late arrival; however, if the Provider chooses to allow the visit to continue despite the late arrival, the visit will end at the original scheduled time to end and the charge will still be 100% of the cost of the visit.
All services rendered at the time of the appointment, even partial, are non-refundable.
If frequent reschedules, cancellations, no-shows or late arrivals occur, Functional Medicine providers at Bodhi Clinic/Iris Mental Health and Wellness reserve the right to consider termination of the patient relationship.
FAQ
Are Functional Medicine services covered by insurance?
Functional Medicine providers at Iris Mental Health and Wellness do not bill insurance directly for either conventional or functional medicine labs. The laboratories themselves may or may not directly bill your insurance. You may attempt to receive reimbursement after the completion of the labs from your insurance company (if it is a commercial plan that is not state medical assistance, Medicare or Medicaid) by requesting a Superbill from Functional Medicine providers at Iris Mental Health and Wellness and submitting that to your insurance company. You understand that you are financially responsible for such costs even if you have health insurance, depending on the benefits and coverage limitations of your health insurance policy.
Will I have to fill out paperwork?
Yes. After securing the appointment, you will receive an Intake Packet via the patient portal that needs to be completed no less than 48 hours before the appointment time to ensure that the Provider has the ability to review the information. Failure to do so will result in an automatic rescheduling of the appointment.
